About the Role

The Health and Community Link Project is part of the Chatterbox Street Outreach service, which provides after-hours assertive outreach for young people who are aged between 12 and 25, who are street frequenting, homeless, or at risk of homelessness and/or engaging in risk taking behaviour.

 

Health and Community Link - Overview

Health and Community link Program - Whitelion Youth Agency in collaboration with St Kilda Gatehouse will provide health support and case management to young people who are street frequenting, homeless, at risk of homelessness, and women engaged in street-based sex work, through mobile outreach and in-hours services. The service is funded by North Western PHN.

 

Purpose of Health and Community Link – Mental Health Nurse (0.5 EFT)

The Health Link and Community Link – Mental Health Nurse will work alongside a team of Outreach Workers to alleviate issues associated with homelessness, and ensure young people (especially young women) can access mental health, sexual and reproductive health services, respond to issues associated with drug and alcohol misuse, family violence, and have access to employment and other day programs.

The Mental Health Nurse will also provide mental health and generalist health based interventions to young people accessing the program as well as provide secondary consultation and practice guidance to the outreach team.

Whitelion is seeking an experienced, qualified and motivated person to work in the Health and Community Link Project. This role would suit an experienced Registered Nurse with a strong commitment to working with young people who are street frequenting, homeless, at risk of homelessness, and women engaged in street-based sex work.
